How do I collect vitals information prior during the online checkout so the information gets pre-populated in the forms?

We’re always working to improve the family’s user experience. One piece of the feedback we’ve been receiving is that the family has to fill in duplicate information over multiple forms.

Since the vitals form was sent at the same time as the other documents, the vitals information was not able to pre-populate the forms, resulting in families having to fill out the same information multiple times.

We’ve updated the process to move Parting Pro’s standard vitals form to be available for a family to fill out immediately after they complete the online checkout. This new experience enables the information collected in the vitals form to pre-populate the remaining forms, removing the issue of families having to fill in duplicate information.

To access the vitals form at the end of the online checkout, the family must first go through the online checkout as an at need case and pay.


The family will then go to a page that lets them know that they need to fill out the Vital Statistics Worksheet.


Clicking “start now” will take them to the form to fill out the Vital Statistics Worksheet. Once submitted, all the information on the Vital Statistics Worksheet will be saved in Parting Pro. You can then generate and send the other required forms with the vitals information pre-populated.


If the family doesn’t fill out the vitals worksheet upon completing the online checkout, they will receive a follow up email with a link to the Vital Statistics Worksheet.


Like other forms, you can see if a family completed vitals in your dashboard.

We are excited that this new experience will save families valuable time when completing forms. If this is new experience is not something you want your families to go through.
